Job description

The Adult Social Care Service (ASC) is looking to recruit to a permanent 18.5-hour Team Manager post. The 18.5 Team Manager post will be based within the Community North Service.

Following an extensive restructure of Service Delivery, Community North consists of five teams currently based in the City Centre. The teams are part of the Living and Aging Well Service: Living and Ageing Well - includes older adults and people with dementia and those accessing occupational therapy, hospital support, and enablement services. Community teams aligned to Primary Care Network (PCN) boundaries. Homecare contracts will also be structured around the PCNs and aligned to this service.

The teams will be undertaking the case management of older people (over 65) and people with dementia (other than those people with a learning Disability) living in the Community.

Community North Teams will provide this social work service to people living in the north part of Sheffield and will complement a similar service which supports people in the south part of Sheffield.

It is envisaged that the new teams will link closely with local Primary Care Networks and develop good working relationships with local Commissioned Service Providers.
