Role Overview & Responsibilities
An opportunity has arisen for a Band 3 Therapy Assistant to join the therapy team working into Sheffield Intermediate Care Beds service. The service is based across three private sector care homes and provides a short term community based in-patient facility for people who are unable to return home after a hospital admission.
The service aims to enable patients to return home with appropriate support as early as possible by providing a range of person centred reablement and rehabilitation as well as complex discharge planning intervention.
Each unit has an MDT with input from Occupational Therapy, Physiotherapy, Therapy Assistants, Nurse Case Manager, Consultant Geriatrician, GP, Nurses and Support Workers. The successful candidate will be based in 1 unit but they may be expected to work flexibly across the units, providing cover where/ if required.
Experience in the rehabilitation of older people, experience of working in an intermediate care service and a good understanding of current issues impacting on intermediate care are desirable.
We are looking for a motivated and proactive senior Therapy Assistant with excellent communication skills and experience of working in the rehabilitation of older people and community experience is desirable. You will need to be able to work autonomously, (assessing non-complex patients and delivering treatment plans delegated by clinicians) and as part of a multidisciplinary team.
To assist the multidisciplinary team in the provision of patient centred care.
To implement rehabilitation programmes and complete patient records.
Undertake assessment of patients together with therapists within departmental guidelines.
To assist the multidisciplinary team in maintaining an efficient working environment.
As a Band 3 you will have the support from the Band 5, 6 + 7 Occupational Therapists and Physiotherapist, as well as the other members of the MDT.
