Role Overview & Responsibilities
An exciting opportunity has arisen to join the Continuing Healthcare team at SWL ICB Croydon to lead on the assessment of adult clients with and without a learning disability.
The post holder must have a strong clinical background, assessing and working with clients who have a learning disability, and an in depth knowledge and practical experience of Continuing Healthcare. The individual must be able to assess and plan to mitigate risk, plan care delivery and potential reduction of support, and negotiate with care providers, clients, and their representatives in a professional way. We are looking for an individual who is able to work autonomously, prioritise effectively and deliver targets within a given deadline.
