Role Overview & Responsibilities

Set in the beautiful town ofCrawley, the mainline train station, you will find our community mental health assessment and treatment service. We work with adults living with mental illness, supporting their recovery and the development of skills and strategies to support them to maintain their wellbeing. We are a lively and friendly team where you will be offered plenty of support along with mentorship and supervision from senior nursing colleagues, active management support and the chance to undertake training courses relevant to your professional practice.

As a Mental Health Liaison Practitioner in our service you will be working as part of a Multi-disciplinary team which include psychologists, nurses, occupational therapists and medics.

To protect the health and safety of our staff/workers, patients, and those attending our sites, we encourage our workforce to get fully vaccinated against COVID

You will work with people with mental health problems, providing specialist short term interventions with a focus on recovery and reablement .You will act in the role of Lead Practitioner for a defined caseload of clients. We encourage knowledge sharing and teamwork so all team members are involved with the full range of best practice interventions. You will benefit from regular caseload reviews and we are currently developing family interventions within our service, with plenty of opportunities for you to become involved.

Requirements
  • A Registered Mental Health Nurse or Occupational Therapist
  • Experienced in working with people with mental health problems that would benefit from the clinical interventions and expertise provided by secondary mental health services.
  • A team player who is willing to collaborate and strive for best practice
It would also be advantageous if you have:
  • Experience within a community assessment and treatment service

To work as part of the secondary care mental health service, acting as a link clinician to Primary Care Network teams. Clinical work should be carried out within primary care settings where possible.
To work in partnership with GPs and other primary care colleagues around individual patients and to work jointly with them to develop care plans for the client.
To undertake routine assessments which have been triaged to the role.
To maintain a close working relationship with local secondary care teams, keeping them informed of developments within primary care.
To build professional relationships within Primary Care Networks to support effective development of care pathways within localities.
To work closely with secondary care colleagues, sharing expertise where necessary to ensure a consistent MDT response.
To have an awareness of available local resources and services and how to access these
