Role Overview & Responsibilities
An exciting rare opportunity for an Occupational Therapist to join our Integrated Community Therapy Team. Providing a multi disciplinary approach alongside physiotherapists, therapy support workers, Advanced Clinical Practitioners, nursing services, GPs and Social Care agencies.
The Integrated Community Therapy Teams aim is to develop optimum levels of function and independence in the physical, emotional and social aspects of life. To prevent hospital admissions and facilitate early supported discharge as well as to support longer term condition management.
Enabling the population of Herefordshire to live well within their own homes and accessing their local communities. Encompassing the Care Closer to Home ethos.
We are looking for an enthusiastic, forward thinking Occupational Therapist with an interest to develop clinical therapy skills within Urgent Response, Discharge to Assess and Neighbourhood Team therapy.
We have a high patient satisfaction rate with excellent feedback, also enhancing job satisfaction.
